All patients with Commercial Insurance (not Medicare, not Self Pay) are required to leave a $100.00 deposit for copays and deductibles: cash or credit card only.

This new policy is due to the large number of defaulted patient accounts as of late.

We accept the following insurances:

Aetna PPO only     Beech St.       Blue Cross PPO only   Cigna all

Coventry all          First Health     Medicare*                  United all

Mail Handlers         Humana Gold only (not Humana Medicare)

*We are Non-Participating providers with Medicare.   All Medicare patients pay for their visit at the time of service and are then reimbursed by Medicare directly
